The Bromley Integrated Care Network

Join us to explore how Integrated Care Networks (ICNs) bring together a multidisciplinary team to support patients with complex or long-term conditions. Through a holistic approach, patients are identified by their GPs and assessed at home by community matrons. Their cases are then discussed in collaborative meetings. Together, the team develops personalised action plans to help patients stay well, remain independent, and reduce unplanned hospital admissions.This session will highlight how joined-up working across primary care, community services, and the voluntary sector can improve outcomes and deliver truly person-centred care.
