This training provides health and care professionals with an introduction to the Universal Care Plan (UCP) – a London-wide digital care planning system that supports personalised, coordinated care across services. Participants will learn how to create and update care plans, record patient preferences and support needs, and use the UCP to improve communication and integrated working across health and care settings.
