Invitation to Care Home Learning Network session on Universal Care Plans (UCP) - CLOSED

GPs working with care homes in Lambeth are invited to join the next London Care Home GP Learning Network session, hosted by the Health Innovation Network in partnership with the London Ambulance Service.

Taking place on 14 April, 1:00–2:30pm via Microsoft Teams, the session will focus on “What makes a good Universal Care Plan (UCP)”—a key tool in improving care for residents and reducing avoidable hospital admissions.

Dr Katherine Buxton, palliative care consultant and clinical lead for UCP, will share practical insights on delivering high-quality care planning. The session will also include perspectives from a GP lead, care home manager and paramedic on how effective UCPs can support better outcomes for care home residents.
