Primary Care Networks

Primary Care Networks (PCNs) are groups of GP practices “working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local”.

Here are the PCNs in Lambeth:

Integrated Neighbourhood Teams (INTs)

In Lambeth, and nationally, the NHS is collaborating with local councils, voluntary organisations, and community partners to design and deliver neighbourhood-based health services. This work aligns with the ambitions outlined in the national 10-Year Health Plan.

There are five INTs:

• Clapham
• Brixton and Herne Hill
• North Lambeth & Stockwell
• Norwood
• Streatham
